Athletic Physicals Information

All students in grades 7-12 who wish to join a Ligonier Valley School District PIAA sport team for the 2019-2020 school year must complete the PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) .  


The process in which learners will submit their PIAA Comprehensive Initial-Pre Participation Physical Examination (Athletic Physical Packet) and supporting information has been changed for the upcoming 2019-2020 school year.  We are excited to announce that we are now offering the convenience of online registration and physical form completion through FamilyID.

FamilyID is a secure registration platform that provides learners and families with an easy, user-friendly way to register for all athletics.  When you register through FamilyID, the system keeps track of your information in a FamilyID profile. You enter the information only one time regardless of the number of athletic teams that the athlete participates.

All of the information that used to be tediously entered on the physical packet can now be entered online with the exception of Section 6 (the Medical Examination Form) - this will still be a hard copy that athletes must have completed by a physician.  Upon completion of the actual physical examination, you will be able to upload the form into your FamilyID account by scanning the page or taking a pic of the form.

Your son or daughter will receive a copy of the PIAA SECTION 6 at their respective athletic sign up.  You can also pick up a form in the LVHS or LVMS Office. A copy can also be printed from the Ligonier Valley Athletic Scheduler Website or the LVSD website.

DEADLINE to complete your entire registration in FamilyID (including uploading PIAA SECTION 6) is 11:59 pm on August 1, 2019. You can scan or take a pic of PIAA SECTION 6 to upload to FamilyID. When your registration is complete, you will receive two email notifications:  1. When the registrations have been RECEIVED and 2. When your entire registration process has been APPROVED.


It will be helpful to have the following information handy to allow for accurate completion of your online registration.

(example: Doctor information, Health Insurance Information, etc.)


A parent/guardian should register by going to:

Follow these steps:

  1. To find your program, click on the link provided by the Organization above and select the registration form under the word Programs.

  2. Next click on the green Register Now button and scroll, if necessary, to the Create Account/Log In green buttons. If this is your first time using FamilyID, click Create Account. Click Log In, if you already have a FamilyID account.

  3. Create your secure FamilyID account by entering the account owner First and Last names (parent/guardian), E-mail address and password. Select I Agree to the FamilyID Terms of Service. Click Create Account.

  4. You will receive an email with a link to activate your new account. (If you don’t see the email, check your E-mail filters (spam, junk, etc.).

  5. Click on the link in your activation E-mail, which will log you in to

  6. Once in the registration form, complete the information requested. All fields with a red* are required to have an answer.

  7. Click the Continue button when your form is complete.

  8. Review your registration summary.
